Job Highlights:
This well-established ambulatory clinical pharmacist specialist position is embedded directly in our SSM Health WI- Dean Medical Group Infectious Disease Clinic where the pharmacist works alongside the other providers and is an integral member of the care team. This position focuses on safely and effectively managing our patients on Outpatient Parenteral Antibiotic Therapy (OPAT) as well as supporting and co-managing our patients with HIV and PREP therapies utilizing broad collaborative practice agreements that allow a high degree of autonomy. The pharmacist autonomously has face-to-face or telehealth visits with patients for HIV and PREP medication management. The ID pharmacist specialist also serves as an expert for immunization questions and initiatives and will be involved in ambulatory antimicrobial stewardship initiatives.
This is a 0.5 FTE job share position with another specialist and you will be expected to be on site in clinic the majority of the days you work but can accommodate some flexibility with remote coverage when necessary.

Job Summary:
Assumes an expanded pharmacist role by providing direct patient care and may practice with approved privileges granted through collaborative practice agreements with Medical Staff. The role will be actively engaged in all complexities of patient medication management, monitoring, and education, as needed. Participates in the training and education of students, residents, and physicians as applicable.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Gathers necessary data to promote patient safety by ensuring that each relevant medication is indicated, effective, safe, and that the patient is adherent to therapy
- Improves patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.
- Assesses relevant patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues as needed. May design a drug therapy plan for patient-specific problems and independently implements the plan.
- Provides enhanced patient care through detailed counseling on medications and disease states, adherence assessment, medication cost analysis, review of immunization status, and communication with patient’s providers and pharmacy.
- Provides expert consultation with other health care professionals in the development of drug-related treatment plans, assessment of drug therapy, outcomes and identification and resolution of drug-related problems.
- Identifies patient outcomes, correlates those outcomes to clinical processes, identifies processes that yield best outcomes, and assists in the implementation of those best practices.
- Participates in quality management programs to facilitate continuous process improvement initiatives and quality improvement projects.
- Precept pharmacy students and residents, as applicable.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
EDUCATION
- Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy
EXPERIENCE
- Two years experience or completion of PGY1 residency
PHYSICAL REQUIREMENTS
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
- Frequent keyboard use/data entry.
- Occasional bending, stooping, kneeling, squatting, twisting and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Rare climbing.
REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS
Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment. Other resuscitation certifications may be required dependent on department.
