JSSA is looking for a full-time field RN to join our hospice team and support the program with our transitions program servicing end of life patients residing in Montgomery County.

Responsibilities:
• Identify and evaluate palliative needs of patients, utilizing the nursing process
• Provide check-ins (in-person and telehealth) to recent hospice discharges and those with advanced illness that are not eligible for hospice services.
• Work closely with the Hospice Intake department, Hospice Social Workers, Liaisons, and other key hospice IDG team members to assure compliant, quality and timely care.
• Work closely with other JSSA departments to help clients secure comprehensive and coordinated care that may be available within the Agency.
Qualifications:
• Active RN license to practice in Maryland.
• Minimum 1 year of Hospice or Homecare experience in a clinical role.
• Experience managing patients with complex illnesses.
• Highly effective verbal communication skills including courtesy, resourcefulness, and efficiency in answering questions, giving directions, locating staff and explaining hospice/departmental policies and procedures are necessary.
• Proficient technical skills in telemedicine, Electronic medical records, and Microsoft Office 365.
• Comfortable with navigating the home care, assisted living and nursing home environments.
